North Texas Pams Chapter

The North Texas Chapter of the Peruvian American Medical Society (PAMS), a 501 (c) (3) non-profit organization, is planning the 5th Annual Medical Mission Trip to Ayacucho, Peru on August 8th-12th. The Mission trip will be led by Dr. Pablo Uceda, a native Peruvian, whose vascular and general surgery practice is based in South Dallas. Dr. Uceda leads a volunteer multi-speciality team including general surgery, vascular surgery, plastic/reconstructive surgery, anesthesiology, pediatric medicine and dentistry. The surgeons and other clinicians will be supported by a team of nurses, surgical technicans and other allied health professionals.

On July 15th, the North Texas Chapter of PAMS will hold a gala fundraising event at the Mediterranean Villa, 1111 Wet N Wild Dr. Arlington, Texas.

Please visit the rest of this website for more information on the organization and the mission trip. All money raised will go directly to the care of the indigent patients of Ayacucho. Ayacucho Mission volunteers do not receive compensation for travel expenses or for services they provide.

In order to make our gala and mission trip a success, we are offering several levels of sponsorship. The levels of sponsorship are as follows:

Presenting Sponsor- (Exclusive) cash/or in kind gift of $5,000 or more – Opportunity to give a presentation of 15 minutes at the gala. Full color page ad on the event program. Company or private name and logo as a stand alone, during the opening slide of the video presentation to be aired at the gala. Signage on all printed and web marketing materials for one year, one centrally-located tables, with seating for 12 during the gala. (Filled as of 5/31/2011)

Gold Sponsor - (Maximum of 3) cash or in kind gift of $2,500 to $4,999 - Full color page ad on the event program. Company or private name and logo on the second slide of video presentation to be aired at the gala event. Signage on all printed and web marketing materials for one year. One table of 10, centrally located during the gala. Informational booth at event (optional upon request).

Silver Table Sponsor - cash gift of $1,250 - Half color page ad on the event program. Company or private name and logo on the 3rd slide, full signage on all printed and web materials. Reserved seating for 10 at the gala event. Informational booth at the event (optional upon request).

Bronze Sponsor - cash gift of $1,000 - Company name and logo on the event program. Company name and logo on the 4th slide of the video presentation. Name signage on all printed and web materials. Reserved seating for 4 at the gala event.

Copper Sponsor - cash gift of $500.00. Company or private name on the event program. Company name and logo on the 5th slide of the video presentation. Reserved seating for 2 at the gala.

In order for your personal or company name and logo to appear on the event program, all tax deducible donations for committed sponsorships must be submitted by June 30.
